Red-card penalty? Empty Red-card penalty?

If a player is red-carded and ejected from a match, can the offense be so bad that the player is suspended for more than one game?  If so, is this the ref's call?  Or is it FIFA's call?

Usually a direct red card results in a 3 match suspension. But since it's the world cup and tournament itself is very short, players usually get 1 match suspensions unless the offense (foul) is horrific, in which case some referring panel decides the longevity of the suspension.
