Maybe it's not a coincidence. Somewhere along the line, something must be going internally wrong with Dortmund
Schalke I have no idea, I only ever watch them in the odd European match
But I do notice they always seem to be missing players as the commentary points out.

Dortmund play with a lots of intensity and run a lot as well, but perhaps it has to do with the medical team with some of these clubs? When a player isn't at 100% they perhaps still give them the good ahead to play?

Well, for now the Schalke guys decided to keep Leon Goretzka another 2 month out as they want to ease him slowly in for now. So maybe they have learned?

Dortmund just lack depth, that means they play some guys too much, which in return makes them more injury prone; which means more injuries, offsetting attempts to add depth, putting more strain on the replacements, making them more prone to injure themselves.
What Dortmund needs is a solid 3-4 months of no football, but that's not a thing that happens.

Leverkusen were leading 1-0, hit the post 3 times and missed 3 CCC. Werder Bremen tie with the only chance they had in the game in the 45th minute

It's not even the highline alone, when they are attacking they attack with all the players bar 1/2 + GK. And the individuals are average as you said.
